All are invited and encouraged to participate in the Disciple Maker Index Survey

This survey will provide you the opportunity to reflect on your own spiritual growth while helping our parish plan for the future. This survey is anonymous and should take 10-15 minutes to complete. 

Step 1 – Complete the survey online – it’s the easiest and quickest way to participate. If you need assistance, we offer a 3-step support plan:
 
1. In-Office Help
You may reach the parish office Monday through Friday from 9:00 AM to 5:00 PM.
 
2. After Mass Support
See the Deacon at the rear of the church after Mass for help with the survey
 
3. PDF Survey Packet
· Request a PDF version to print at home
· Please provide your name and email to receive it
· Return the completed survey to the parish office at your earliest convenience
